To all conference participants:

Dr. Bruno Grbac and the Macromarketing Help Team need to learn  various logistical information pieces including: flight date, time of arrival and departure, flight number or train number, presentation equipment needs, etc.  The link below contains the form that we request that you please fill out:

Transfer, Contact, and Support Form 

(**Note: The document is currently saved in a Word 95/rtf format.  Clicking on the link from Netscape Communicator will bring the option to save the form to disk or open it directly.  If you are using Internet Explorer a login and password menu may come up, click cancel if this prompt arises and the document should pop up easily.  Should you have any problems accessing the form, please send an e-mail note to bets@asu.edu).

In addition to posting this on the web page, we will send an e-mail note to all participants, once the final program is put together, to gather any needed additional information.
